You can find the Dead Ghost at the end of Prison of Elders Level 41 as you make your way down to the treasure room.
How to obtain it:
1. Finish PoE and begin your descent to the treasure room.
2. Stop at the second platform down with a door.
3. Look up, the Dead Ghost will be hanging from a small net below the platform above.
4. Jump up and revive it.
